Completed forms downloaded from this site can be submitted to any Student Centre, by mail or by facsimile.
In particular, it is important that you ensure that your personal details are current on Student Connect.
Note ACU National will not process forms that have been altered in any way.

Enrolment Forms
applications for credit, cross-institutional enrolment, addition of units, change of major, withdraw from course, transfer from a dual to a single degree, waiver of late fee, course complete and right of correction.
Examination Forms
application for deferred examination, notification of examination clashes, special adjustment/s – examinations and special consideration.
Fee Forms
applications for extension of time to pay fees, waiver of late fee, refund of fees, re-credit of student learning entitlement, reinstatement of enrolment, permanent residency transfer.
Review and Appeal Forms
