Note: Currently we are converting the forms to enable web based submission. At present, most forms cannot be completed online and are required to be downloaded. Please follow the instructions provided upon clicking on the link to your required form.
Completed forms downloaded from this site can be submitted to any Student Centre, by mail or by facsimile.
In particular, it is important that you ensure that your personal details are current on Student Connect - Managing Your Studies.
Note ACU National will not process forms that have been altered in any way.

Admission (Application) Forms
To apply to study at ACU National you will need to lodge an online admission application via the appropriate admission pathway; ie via QTAC, UAC, VTAC or direct to ACU National. Visit the Course Browser to find course information, including how to apply.
The only exceptions to applying online are ACU National students seeking readmission following exclusion or termination of studies. These students are required to submit a paper based application, using the appropriate form listed below.
